that questionaire from secondary school student i believe.

Some people told me CEA sitting on cases n taking their own sweet times to do things , don't know how true.

"This study aims to determine and evaluate the effectiveness and impacts on the establishment of CEA"......... to who?

u need to consider the different stakeholders and design the survey more specifically. Being too general may suggest lack of depth n knowledge of subject matter.

just submited the form. good form but need more questions ( 3 to 5 more) and add an optional personal comment (if any ) field.

I tried to simplify my survey as much as possible so as to reach out to the general public. I understand that the questions does not meet your standard, but nevertheless, each of it comes with an aim.
